We administer the AHRQ Survey on Patient Safety™ for hospitals, nursing homes, medical offices, ambulatory surgery centers, and pharmacies. We’ve also developed culture assessments that align with AHRQ surveys for emergency medical services and home care. Select one or any number of surveys so you can get the valuable feedback you need.

Relax! We’ve got this. CPS believes the safety culture assessment process is more than just administering a survey. It’s about how your employees can speak to an organization’s leaders through a standardized platform. CPS offers full administrative support, consultation and education services from the beginning of your journey. You don’t need to learn a new software, become an analytic expert, create your own marketing materials, or develop your own education. We’ll guide you through the entire process and make sure you get actionable feedback that can lead to positive change.

We are with you. We walk with you through the entire process. You can be as involved as you want to be. Our goal is for you to have a successful assessment by getting the correct information with actionable insight to make change.

Pick your own dates. You get to choose when your survey starts and ends. We’ll extend your survey if needed to make sure you get your target response rate.

Any device. Anywhere. By default, surveys are accessible from any device. With CPS, organizations achieve more than a 10% increase in their responses over previous methods, and average a 70% response rate.

Get the reports you want. We provide all the reports you want and need – just ask! There’s no additional cost – and no need to learn new software to run reports or access comparable data.

Our survey is provided on a secure (https:) website. Additionally, CPS is a Patient Safety Organization (PSO), and we understand how important confidentiality is to you and your organization.

Industry-leading reports. Our reports are the best in the industry with demographic details, benchmarks, and next steps. They’re provided electronically, so you can save them to any computer, server, or flash-drive to refer to them later. Additionally, electronic files include hyperlinks to helpful resources.

Select only the surveys you need. Chose from hospital, nursing home, home care, medical office, pharmacy, ambulatory surgery center, and emergency medical services.

Save time and money. On average, CPS clients spend 3-6 hours over a 3-month period. Compare that to the 30-60 (or more) hours it can take to administer the survey and analyze the results on your own!

Q     Can the Center assess and support our culture change even if we have not participated with the Center in any other projects?    

A   Yes, we administer and support the assessment process anywhere, to hundreds of organizations across the country - there are no geographic boundaries.  We work with provider groups on a variety of safety projects, culture assessments and consulting.

Q     What support is provided after the survey period?

A   Once we complete your feedback reports and send them to you, we'll schedule your first call. You'll talk with certified patient safety experts from the CPS team who understand your organization's environment. We'll talk through successes and opportunities in your results and help identify causal factors. We'll also work through next steps to make improvements. Additional support is available including onsite training and education, consultation and/or bi-weekly and monthly calls to promote action and retain focus. Your success is important to us.

Q     How much time is required for me to implement the survey at my facility?

A    We've developed a plan that requires minimal resources on your part. We have a few criteria, but in total, only about 3-6 hours over the course of the survey period is expected.  We provide you with customizable email templates, posters, reminders, and more. After the survey is over, we provide you with detailed reports, next steps, and consultation support for making changes. You don't need to learn new software or become an expert analyst - we do it all for you.

Q     What IT needs are required to administer the survey?

A   There are minimum recommended system requirements and web browser configurations. Both are minimal but necessary to access the survey.  We'll work with you and your IT staff to ensure functionality before the survey starts.

Q     How is the survey distributed to the organization?

A   The Center provides you with a custom link (url) or links available for use on any computer or mobile device.  The link can be placed on your intranet, distributed via email, displayed on posters, etc. We also offer sending the link directly to employees via email.

Q     Is the organization's raw data available after the survey?

A    Yes, at the request of the organization, we will provide an Excel workbook with raw survey data and a data dictionary for interpretation.

Q     What type of review is done to ensure the reliability of the data?

A   The Center's experts scan every incoming survey response manually and electronically for discrepancies, duplicates, blanks, and "quick" responses. If a survey respondent chooses all 5s or all 1s, they are not likely reading the questions, since many are reverse-worded, and leaving these responses in can negatively impact your organization's overall results.

Q     How long does the survey period last?

A   We recommend the survey cycle run for 4 weeks, however, if survey responses are low in one department or facility, the close date can be extended, or if 100% participation is achieved before the end of the survey period, the Center can close the survey early to expedite report processing.

Q     How often should we administer the survey to our staff?

A   AHRQ recommends administering the survey every 12-18 months.

Q     Are comparative results included in the reports?

A   Absolutely.  The Center uses AHRQ's most current comparative database for percentile rankings for surveys in hospitals, medical offices, pharmacies, nursing homes and ambulatory surgery centers. For the home care and EMS survey, the Center maintains and incorporates its own benchmarks.

Q     Is the Center's pricing reasonable?

A   Yes, the Center intentionally makes the survey affordable for everyone to encourage participation. All CPS Patient Safety Organization (PSO) participants are eligible for a 20% discount off the standard pricing.  Additional discounts may be available.  Survey rates start at $895 per survey cycle. CPS partnerships offer special rates to clients and members of the following organizations:

Q     Will the results be shown with historical trends?

A   Yes, we provide historical trends on all reports.  Even if you didn't use CPS services in the past, just send us a data file or PDF of your previous results and we will upload them for incorporation into the reports we provide.
