The Center for Patient Safety (CPS) administers the AHRQ Nursing Home Resident Safety Culture Survey. Dimensions and questions are formulated to gather feedback from your staff about the culture of resident safety in your organization; and our feedback reports provide you with next steps to take in your journey.

CPS works with providers across the continuum of care, but this survey is specifically designed to interpret the culture of nursing homes.
